Support for Every Stage of Aging

Senior Care

🩺 Chronic Disease Management

We help adults 65+ manage high blood pressure, diabetes, high cholesterol, heart failure, COPD/asthma, kidney disease, osteoporosis, arthritis, and atrial fibrillation. We begin with a full review and a simple, personalized plan with clear goals for blood pressure, blood sugar, breathing, weight, bone health, and symptom control.

We simplify medications, schedule regular check-ins, and use home tools when helpful (blood pressure cuff, glucose meter, pulse oximeter), with plain-language daily instructions and what to do if symptoms change. After any ER or hospital stay, we follow up quickly to keep you stable so you can maintain steady control, avoid flare-ups and hospital visits, and stay independent.

🏥 Physical Exams

We review your medical and functional history, discuss goals and concerns, and perform a head-to-toe exam tailored to aging. This includes vital signs with orthostatic blood pressure, vision and hearing, oral health, skin, heart and lungs, abdomen, and a focused neurologic and musculoskeletal exam.

We check balance, gait, and fall risk with Timed Up and Go, and assess strength and flexibility in the hips, knees, and spine; we also review cognition, mood, sleep, pain, continence, nutrition, home safety, mobility supports, and caregiver needs. You leave with a clear plan that summarizes findings, priority next steps, and your follow-up schedule.

🧪 Diagnostic Lab Testing

Lab tests provide timely, actionable results to guide care and prevent complications. Core panels usually include a complete blood count; comprehensive metabolic panel; kidney and liver function; A1c and fasting glucose; lipid profile; thyroid function; vitamin D and B12; and a urine albumin-to-creatinine ratio.

When needed, we add condition-specific tests such as BNP for heart failure, INR for blood thinners, iron studies, or inflammatory markers. We order evidence-based screening labs using shared decision-making, then review results promptly with clear next steps, medication changes if needed, and follow-up timing.

Samples can be collected in our clinic with quick turnaround. You receive a simple summary explaining what your numbers mean and when to recheck.

💊 Medication Optimization

We review every prescription, over-the-counter product, and supplement to be sure each is necessary, safe, and working well together. We reconcile your full list, check for interactions and duplications, and use evidence-based tools such as the Beers Criteria to flag high-risk drugs. Doses are adjusted for kidney and liver function, and we simplify regimens to lower pill burden, side effects, and adverse events.

When appropriate, we stop medicines that no longer help, substitute safer options, and align choices with your health plan formulary to reduce cost. We support adherence with clear instructions, refill synchronization, packaging options, and caregiver coordination.

After any ER or hospital visit, we repeat reconciliation and update your plan. You leave with an easy-to-follow medication list, monitoring goals, and specific warning signs to watch for so your treatment stays safe, effective, and manageable.

🌟 Wellness & Preventive Care

We help you stay healthy, active, and independent. Your plan lowers risks for heart disease, stroke, falls, and frailty with nutrition guidance, safe activity goals, weight management, sleep support, and tobacco cessation if needed. Vaccines are reviewed and updated on schedule, including flu, COVID-19, pneumococcal, shingles, and Tdap.

We support bone health with calcium and vitamin D guidance and weight-bearing exercise. We provide fall-prevention tips for home safety and balance training resources. You leave with clear next steps and a simple follow-up timeline so prevention becomes part of your routine.

Senior Care

🎗️ Cancer Screening & Risk Assessments

We identify problems early so treatment is simpler and outcomes are better. We review your history, family risks, and prior results, then recommend evidence-based tests with clear timelines. Typical screening includes stool tests or colonoscopy for colorectal cancer, mammograms as age and health indicate, low-dose CT for lung cancer when smoking history meets criteria, and PSA discussions for prostate cancer using shared decision-making. We confirm whether cervical screening is still needed and consider skin checks or other targeted exams when risks are present.

You receive practical guidance to lower risk through vaccinations, nutrition, activity, and smoking cessation, plus reminders for when each test is due. Results are explained clearly, and any abnormal finding leads to prompt next steps, coordinated follow-up, and referral to oncology specialists when needed.

Senior Care

🧠 Cognitive & Mental Health Assessments

We check memory, attention, language, and thinking using validated tools such as the Mini-Mental State Exam (MMSE). We also screen mood, anxiety, sleep, and substance use with simple questionnaires. We review medications for side effects that affect thinking and look for reversible causes such as thyroid or vitamin problems. Safety, driving, fall risk, and daily functioning are assessed. You receive clear results, a practical plan for brain-healthy habits, and guidance for caregivers.

When needed, we arrange further testing or referrals to neuropsychology, neurology, or psychiatry and connect you with community resources. Regular follow-up tracks changes over time and adjusts care as needed.

Senior Care

🦵Joint & Arthritis Care

We focus on reducing pain, improving mobility, and protecting independence. Your visit includes a focused exam of painful joints, a review of daily activities, and simple tests of strength, balance, and gait. We begin with proven steps: activity changes, low-impact exercise, physical or occupational therapy, weight management when appropriate, and heat or cold therapy. Medicines are chosen carefully for seniors, starting with acetaminophen and topical options. We use anti-inflammatory drugs only when the benefits outweigh risks to the stomach, kidneys, and heart.

When needed, we consider joint injections for targeted relief and order imaging to clarify the diagnosis. You receive a clear home plan with exercises, instructions for flares, and fall-prevention tips, plus help with braces, canes, or walkers if useful. If pain persists or joint damage is advanced, we arrange timely referrals to rheumatology or orthopedics to discuss advanced options, including surgical evaluation.

Senior Care

⚕️Specialist Involvement

When advanced care is needed, we arrange timely referrals and stay involved at every step. We send your history, medications, labs, and imaging in advance, confirm that referrals fit your Medicare Advantage plan, and help with authorizations and scheduling.

After each visit, we review the specialist’s recommendations, update your care plan, and explain next steps in clear language. If procedures, infusions, or advanced imaging are recommended, we help you prepare, manage medications, and arrange follow up. Our goal is seamless, team-based care that reduces delays, prevents duplicate testing, and keeps your primary clinician at the center of your care.
