Specialists are typically contracted and credentialed (including a W-9 form). Primary care physicians complete a consultation first to review eligibility, panel transition, and capitation terms for joining MD Senior Care Medical Group.

Note for PCPs: we currently only accept Medicare Advantage patients enrolled with SCAN Health Plan, Alignment Health Plan, and Imperial Health Plan.
